Here is my list of the hardest beat'em'ups ever played.
These sofware houses should know that "normal" difficulty doesn't mean "hard" difficulty and "hard" difficulty doesn't mean " call an ambulance ".
- Art of Fighting 2
the prequel was fairly hard, but the second episode is just way to glitchy and weird as regards control. AI just power-abuses of any weakness even in Level 1 difficulty. What a waste for a legendary series, being f***ed up at the second episode.
- Virtua Fighter 4 Evo
I can understand losing in arcade mode... at Hard or worse difficulty. But try not losing at Normal. The whole series is for schizos. I love it, but I hate it.
- Kof Maximum Impact 1/2
Ok SNK you did a great job in making a 3d version of KoF, but could you at least spare me some episodes of epilepsy by trying too hard with your game... also: so boring, you have to complete 400 missions and finish the game with any character 16 times in order to fully unlock the game? Come on, you make me wanna cheat! (and if any of you knows AR Max codes for PAL version, you're my best friend).
- Dead Or Alive ( the whole series)
Another game you must be a schizo to be good at. Really too fast gameplay, and those bouncing breasts make it even more difficult.
- Samurai Showdown
I really don't get this game. I like Last Blade, at least some commands are understandable and by retrying and retrying you can learn actually how to beat the game and meet Kagami (funny character, I've already seen you somewhere...).. but Samurai Showdown has a really weird control system and brutal AI after the 3rd stage. Too bad it is well drawn.
More than these I know other arcade games which just coin-sucked for their insane difficulty.
I know you should be at least good at something but, hey, requiring so much from a gamer turns a game into something too serious.
I'll add more once I finish each game.

Those aren't beat'em ups
-
maximum carnage is my favorite beat em up and was pretty hard for me
-
The Battletoads games are pure tough. But the arcade game was a quite piece of cake for me.
-
SFII on hardest. Couldn't even get past the first round after at least 20 quarters.
-
yeah those are considered VS games that you mentioned more so actually. Beat em ups would be. double dragon. Captain Commando. Xmen arcade. TMNT: Turtles in time. Mutation nation. Final Fight. Streets of rage blah blah blah lol
But anyway the toughest beat em up in my book to remember?
Ninja gaiden arcade. And yeah I agree with Rabbitxpp heavy on Maximum carnage. sheesh....loved the music but damn that was tough!
-
Oh, music in Maximum Carnage is great indeed, love it  About it's difficulty... to be honest, I don't remember :P I played long ago, but listening to VGMs from time to time.
Regarding Battletoads - ohhh I agree so agree, hard as hell at times, I won't say name of level that starts with T T and the other one C W so you won't have nightmares remembering those, lol BTW there's hack of it that adds Dragons(Jimmy and Billy I mean not any other ones) to BT game and adds alters other stuff.
As for hard fightings or VS games, hmmm, I'd say Virtua Fighter 4 from those I played. Had rough time with it.

He clearly meant fighting games and not beat´em ups, no major foul there.
Anyway I can´t think of a fighting that was really that hard.
-
Duh, of course it was obvious, but we used a chance to talk about beat'em'ups too as the title suggests...
Removed file host link for BT&DD - On Ragnarok hack, now link grab the patch is here http://www.romhacking.net/hacks/2973/ Though it's updated to v1.2 it's still may be not final for other bugs might be spotted and fixed or something altered. Have a good time with it :D
-
Hey Captain Obvious, bring it down a notch
-
Aw, come on :-j don't be like that [-X
But seriously, nobody ever had any struggles in fightings? Most issues with fighting game I had was with VF4. Accustoming to the system is pain^3.
